Clinical Labeling
Labeling includes a minimum of 2 patient identifiers. Acceptable identifiers include patient’s full name, date of birth, account number and requisition number. If specimens are submitted with a Pathologists’ Regional Laboratory requisition, a numbered sticker that is unique to every requisition is provided and is acceptable as a unique identifier. The specimen or requisition should also include date and time of collection. Specimens without 2 definitive patient identifiers will unfortunately be rejected and must be recollected. Please ensure that all specimens submitted to PRL for testing are properly identified.
